Office Visits

Bring your smile; but also remember to bring these things with you to your first dental appointment:

  • Your completed Health History Forms
  • Your insurance card and photo ID
  • Current X-rays - if available

Our practice is committed to providing you and your family with safe, gentle, and state-of-the-art dental care. We understand that you, or your child, may feel anxious about visiting the dentist; you are not alone, and our practice puts forth an extra effort to make sure that you are comfortable and happy every time you visit our practice. We are sensitive to your needs as a patient; you can always count on receiving individual attention from our doctor and staff.

Your first visit is all about you your comfort, your happiness, and your dental health.

Your first visit with Dr. Pawlowski includes:

  • A review of your complete dental and medical history forms.
  • Complete X-rays of your teeth and mouth.
  • An oral exam - Dr. Pawlowski will chart and examine each tooth, make sure your teeth and gums are healthy, and check for any bone loss, tissue loss or early signs of tooth decay.

Once Dr. Pawlowski has reviewed your dental and medical health history forms, and completed your dental exam, he will provide you with a dental consultation, and information about any additional dental treatments you, or your child, may need. During this time Dr. Pawlowski will also answer any questions you may have. Dr. Pawlowski and his staff welcome any questions you may have, and are always available to listen to your concerns.



Regular checkups are an important part of maintaining your healthy smile!

No matter what your age, visiting the dentist on a regular basis will help keep your teeth and gums healthy, and your smile bright!

The American Dental Association recommends that you, and your child, visit the dentist at least every six months for regular checkups and every three to six months for teeth cleanings, depending on your needs; and, we would love to see you here at least every three to six months to make sure that your smile is staying healthy!

When needed, we coordinate care with your dentist if you were referred from their office. We work well and communicate effectively among our referring outside dentists and specialists so you receive the best care possible. We recommend that you see a general dentist at least once a year. If you were referred by a dentist or if we have referred you to one of our referring dentists, we coordinate cleanings on an alternating basis to be sure that you are taken care of in the most comprehensive fashion.

We understand that you have a busy schedule, so while we like to make sure your appointments are informative, we also like to make sure they don't stop you from doing the things you love! When preparing to visit the dentist, please know that your appointment can last from 20 to 60 minutes.

Dental Insurance Information

We accept all dental insurance with few exceptions such as medical coupons. Also we're preferred with the following companies:

We are happy to estimate your coverage and bill your insurance company on your behalf. We do expect you to pay your estimated portion for services as they are completed. When and as a contracted provider, you are only responsible for the patient portion shown on the explanation of benefits. This means savings for you and your family. As always, insurance benifits are determined by your insurance company as per the agreement between your employer and the insurance, you are always responsible for any remaining portion. We strive to be sure that your portion is as low as possible and will provide pre-determination assistance as requested to help with estimates.
